Root Canal: Endodontics

 

Endodontics deals with the health of the nerves & blood vessels of teeth.

Root canals are the most common procedures relating to endodontics. When a tooth becomes infected it is usually related to the nerves and blood vessels of the tooth called the pulp chamber and internal root structure of the tooth. The infected nerves need to be removed. If left untreated an infection can turn into an abscess, which is a much more serious problem that includes bone loss in the jaw and probable spreading of the infection into the soft tissue surrounding the tooth.

The area around the tooth is numbed with a local anesthetic to start the procedure. We  then drill down into the tooth's pulp chamber to create an opening into the canal(s). 

We then are able to remove infected tissue and clean the canal. After the infection has been removed, the space if filled with a sealant cement and master points with the material called gutta percha.

It is highly recommended that a tooth that has undergone a root canal is fitted with a crown. This will improve the appearance of the tooth, and will also make it much more likely that the root canal is successful and that the tooth will not fracture since the tooth is not alive and has no blood supply leaving the tooth prone to breakage and loss.  All root canaled treated teeth shoul have crowns, but molars and premolars are exceptionally prone to fracture after root canal therapy compared to front teeth, usually.

"Root canal" has become a way our society describes something that is absolutely terrible.  This is not the case today.  Most root canals are completed withouty any discomfort and the success rate is nearly 100%.  I have had many patients tell me during my 33 years in dentistry that they had a root canal and still lost the tooth.  Nearly 100% of the time the loss was due to not getting a crown and the brittle root canaled tooth fractures beyond repair and must be extracted.  It is therefore not the root canal that was the problem, it the the problem of patients not following up withe a crown.

Local anesthetics and proper pain medication allow the procedure to be performed with little to no pain in most cases. There may be some soreness following the procedure, but that is normal for most dental procedures. Over the counter painkillers are usually enough to relieve any pain afterwards, but we may prescribe medication for the infection and pain. The procedure will also relieve you from pain caused by the infection allowing you to enjoy all the foods you love without any pain from heat, cold, or biting too hard.
